Our Services
 
Personal Care Homes (PCHs) are residences that provide shelter, meals, supervision and assistance with personal care tasks, typically for older people, or people with physical, mental health, or cognitive disabilities who are unable to care for themselves, but do not need nursing home or medical care and prefer to reside in a community atmosphere that allows independence. While available services vary and are based on the individual needs of each resident, services provided at a typical PCH include assistance with a variety of daily living tasks. Serenity Care's facilities provide daily living, health, and social services as follows:
*24 hour awake staff support
*individualized assessments and support planning based on a person-centered team approach
*three nutritionally balanced meals daily-tailored to your needs (see our sample menu!)
*special attention paid to medical diets such as for those with renal failure or Diabetes      
*snacks twice daily and as needed per special dietary requirements                                  
*full laundry services                                                                                                   
*housekeeping services     
*diabetic care including insulin injections     
*shopping                                                                  
*managing finances including representative payee services
*assistance with setting appointments and obtaining transportation services
*medication management using a state of the art computer based system
*recreational activities including music therapy and cognitive exercises for Dementia
*daily exercise                                                                                                                            
*in-house PCP, Podiatrist, Psychiatrist, Psychologist and Psychiatric Nurse
*personal hygiene and haircutting services
*incontinence care     
*coordination with outside healthcare agencies to ensure continuum of care
*ability to contract with outside physical, occupational and speech therapists, and nursing care
*ability to contract with home health and hospice agencies
*ensure continuity of care via short & long-term care planning
*behavioral support for MH/MR and Dementia residents
*referrals to outside healthcare agencies and therapists
*assistance with applying for Veterans benefits
*assistance with Social Security and health insurance claims 
*respite care and short term stays offered based on availability of space

*All staff receive a minimum of 40 hours of training in their first year and a minimum of 20 each subsequent year. We exceed the DPW requirement of only 12 hours of annual training.                                                                                                                                                                                                                                                         

Pricing is based on market rates for room and board as well as the level of care needed by each resident which is determined through individual assessments. Additionally, private rooms will cost slightly more than shared rooms. We accept Long Term Care Insurance, private pay, Trust payments, Social Security, Social Security Income, Social Security Disability, the Personal Care Home Supplement, and Veteran's Benefits as payment, and will assist you through the complicated application process to expedite your funding. Space is limited for SSI beds, so please call for availability.
